Welcome to SilhouetteRx, LLC (“we,” “our,” “us”).
By accessing or using this website, virtual clinic platform, links, telehealth services, or booking tools, you agree to the following Terms of Service.
1. Services
SilhouetteRx provides virtual healthcare, wellness, and weight-management services for patients aged 18 and older. Services are available to residents of states where our providers are licensed.
Shipping & Delivery

Patients are responsible for providing an accurate and complete delivery address.

If a medication shipment must be resent due to:

An incorrect address provided by the patient

Missed delivery

Delivery issues requiring re-shipment
the patient will be responsible for:

A $20 re-shipment fee, plus

The full cost of the medication

SilhouetteRx, LLC is not responsible for delays or errors caused by third-party shipping carriers.
Third-Party Financing (BNPL)
When available, third-party financing options (e.g., Buy Now, Pay Later services) are offered through independent providers.

Financing approval is determined solely by the third-party provider.

Medical services are not contingent upon financing approval.

Any refunds are subject to the third-party provider’s terms and may exclude financing or processing fees.
2. Medical Disclaimer
All medical advice and treatment are provided by licensed clinicians. Information on this site is for educational purposes only and not a substitute for in-person medical evaluation when needed.
3. Eligibility
You must be at least 18 years old to receive care. By using our services, you confirm that all information you provide is accurate and complete.
4. Payment & Memberships

SilhouetteRx, LLC is cash-pay only.

SilhouetteRx, LLC does not bill insurance and does not provide insurance reimbursement forms.

Fees must be paid at the time of service or through monthly membership.

Recurring monthly fees that bill automatically

No refunds will be issued after a visit has occurred, subject to the Refund Policy below.

Weight-loss memberships include medication

For services purchased in advance through our website, payment is collected at the time of purchase.
Payment Methods
SilhouetteRx, LLC accepts payment through secure, PCI-compliant third-party payment processors, including but not limited to:

Wix Payments (card, ACH, and approved third-party financing options)

Bluefin (card and ACH for recurring billing)

Square (one-time payments only, when applicable)
SilhouetteRx, LLC does not store patients’ full debit or credit card information. All payment information is securely processed and stored by our payment processors.
Card on File Authorization
Patients may be required to keep a valid debit or credit card on file.

A card kept on file will not be charged until the time services are rendered, unless otherwise disclosed and authorized.

By providing a card on file, you authorize SilhouetteRx, LLC to charge the card for services provided, missed appointment fees (if applicable), and authorized charges per this policy.
Appointment Payments & Refunds

Payment is due at the time of the visit.

No deposits are required to schedule an appointment.

No refunds will be issued after a visit has occurred, subject to the Refund Policy below.
5. Telehealth Consent
By scheduling an appointment, you consent to receive care via telehealth. You acknowledge that telehealth involves electronic communication and that all sessions are conducted over secure, HIPAA-compliant platforms.
By using our services, you acknowledge:

You may receive care through secure audio/video communication

Telehealth has limitations compared to in-person exams

You must physically be located in Oklahoma at the time of your visit

You must complete all required consents before care
Electronic communications are not monitored continuously and should not be used for urgent or emergency concerns.
6. Refund Policy

Membership fees are non-refundable

Medications cannot be refunded once ordered. This applies regardless of patient response, preference, or decision to discontinue use.
Chargebacks & Disputes
Initiating a chargeback or payment dispute does not negate this policy.

Documentation of services rendered, signed consents, and visit records may be submitted to payment processors.

Excessive or fraudulent chargebacks may result in dismissal from care.
Cancellations

Appointments must be canceled at least 12 hours prior to the scheduled appointment time.

Cancellations made less than 12 hours before the visit or failure to attend the appointment will result in the visit fee being charged and non-refundable.

Failure to attend a scheduled appointment without proper cancellation constitutes a no-show and is treated as a completed visit for billing purposes.
7. Provider–Patient Relationship
The provider–patient relationship is established only after:

Your ID is verified

Your provider approves your treatment
8. No Guarantee of Treatment
Submitting forms or payment does not guarantee medication approval.
9. No Emergency Services
SilhouetteRx does not provide emergency medical care. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
10. Right to Terminate Care
We may decline or terminate care for inappropriate behavior, noncompliance, falsified information, or safety concerns.
11. Limitation of Liability
SilhouetteRx and its providers are not liable for any damages arising from your misuse of the website or failure to follow medical advice. Your sole remedy for dissatisfaction is to discontinue use of our services.
12. Governing Law
These Terms are governed by the laws of the State of Oklahoma.
13. Updates
We may update these Terms periodically. Continued use of our website or services constitutes acceptance of the updated Terms.
Individual results vary, and no specific outcome is guaranteed. Administrative or processing fees are non-refundable. SilhouetteRx, LLC reserves the right to update this policy at any time. Updates will be posted on our website.
Policy Acknowledgment
By scheduling an appointment, purchasing services, or providing payment information, you acknowledge that you have read, understood, and agree to this Payment, Refund, and Billing Policy.
